The Process Map & Timeline are for you information on  Fresh Start's referral process. 

Read through the Initial Referral Information (PDF 82KB)

Please follow these steps for referral:

Complete the PC with your Fresh Start Principal Representative  Principal Checklist (PDF 73.6KB)

Complete the Student Referral Form (PDF 266KB)

Ensure all documents are sent to your Learning & Wellbeing Advisor in a timely manner.

Please include any relevant Risk Mangement Plans or Student Behaviour documents with the Referral papers.
